If so, we'd love to connect with you. Custom Home Care (CHC) is seeking a Nurse Care Manager/Coordinator to join our team. This role supports primarily independent older adults who benefit from wellness oversight and guidance. Key Responsibilities:

Coordinates care between Client, and/or Client Representative as appropriate. Oversees the daily workflow of Care Professionals. Provides constructive and timely performance evaluations and training as needed. Conduct assessments for new clients and develop individualized care plans (hourly or live‑in) Reassess existing clients and update care plans as needed Educate and advise clients and families on care needs and recommendations Communicate care plans to CHC caregivers and provide ongoing coaching Address caregiver concerns and adjust care plans in collaboration with families and physicians Keep community management informed of any changes in residents' health status Perform routine client visits and monitor ongoing conditions Build and maintain strong relationships with referral sources Proactively identify and engage potential clients in need of home care services Utilize technology effectively for documentation and communication

Skills:

Strong supervisory and leadership skills. Good verbal and written skills to communicate with Clients, Client Representatives, referral sources, physicians, and other health professionals. Must be able to effectively communicate in English. Demonstrated proficiency and confidence in using technology and digital tools with the ability to quickly learn software and related Agency systems Exceptional time management abilities and strong organizational skills

Education and Experience:

Registered Nurse or Licensed Practical Nurse with current Illinois state license. Experience with the geriatric population and the behaviors of Dementia.
