LPN HOME HEALTH
Shift:
- 8 Hours Days 40 hours/week. (5
  • 8) 13-week contract Provide comprehensive nursing assessments, including vital signs, physical examinations, and patient history documentation using electronic health records (EHR) systems.
Administer medications via injections, IV infusions, catheter care, and other prescribed treatments following strict aseptic techniques and safety protocols. Manage complex care plans for patients with chronic conditions such as Alzheimer's disease, dementia, or developmental disabilities, ensuring adherence to long-term care regulations. Perform specialized procedures including tube feeding, venipuncture, sterile processing, and wound suturing as required. Educate patients and their caregivers on medication management, disease processes such as infectious diseases or acute pain management, and self-care techniques. Collaborate with interdisciplinary teams including physicians, case managers, and therapists to coordinate discharge planning and post-acute care services. Maintain compliance with HIPAA confidentiality policies while documenting medical records accurately within EMR/EHR systems. Respond promptly to triage calls or urgent patient needs, providing guidance or arranging immediate interventions when necessary. Experience Proven experience working with diverse patient populations across multiple healthcare environments such as hospital medicine, skilled nursing facilities, or home care settings. Critical care experience including ICU or Level I Trauma Center exposure is highly desirable. Demonstrated proficiency in medical terminology, ICD coding (ICD-9/10), and medical documentation standards. Hands-on experience with IV insertion, catheterization, phlebotomy, and medication administration in accordance with safety guidelines. Familiarity with managing patients requiring ventilator support or specialized equipment like spinal taps or NG tubes is advantageous. Prior experience working with individuals with disabilities or behavioral health needs enhances your ability to deliver tailored patient-centered care. Knowledge of infection control practices and emergency medicine procedures ensures readiness for urgent situations in home health environments.
