Care Coordinator (LPN) - Illinois Based with Support for Indiana Operations Amada Senior Care Orland Park Make a Difference Every Day At Amada Senior Care, we believe that senior care is more than a service—it's a commitment to improving lives and supporting families through every stage of the aging journey. We are seeking a compassionate, organized, and relationship-focused Care Coordinator (LPN) to join our growing team. This position will be based primarily in Illinois while providing support to our Indiana operations. Position Summary The Care Coordinator serves as the primary liaison between clients, families, caregivers, healthcare partners, and Amada leadership. This role is responsible for conducting client assessments, maintaining care plans, performing quality assurance visits, supporting caregiver training and development, and ensuring outstanding client and caregiver satisfaction. The Care Coordinator embodies the values of Amada Senior Care at all times and represents our commitment to professionalism, compassion, and excellence. What You'll Do Client Care & Relationship Management Build trusted relationships with clients and families throughout their care journey. Conduct new client assessments and intake visits. Complete routine client visits to ensure care plans remain accurate and client needs are being met. Identify opportunities for additional services and resources that may benefit clients and families. Collaborate with healthcare providers, case managers, and community partners to ensure continuity of care. Care Plan & Documentation Management Maintain accurate client records within AxisCare. Update care plans and communicate changes to caregivers, families, and referral partners. Ensure all client documentation is completed accurately and timely. Complete required client and caregiver follow-up calls and visits. Caregiver Development & Support Assist with caregiver onboarding and orientation. Conduct hands-on caregiver training and skills development. Participate in employee engagement and retention initiatives. Support caregivers by providing coaching, guidance, and resources. Communication & Coordination Communicate client concerns, updates, and satisfaction feedback to leadership daily. Partner with Recruiting, HR, Scheduling, and Operations teams to address caregiver needs and concerns. Coordinate Meet & Greets, caregiver training sessions, and care plan updates. Support scheduling efforts as directed by the Lead Case Manager. Assist with incoming calls, documentation, and operational support. Serve as a resource for on-call staff to help resolve client and caregiver concerns. What We're Looking For Required Qualifications Current Licensed Practical Nurse (LPN) license or higher. Excellent written and verbal communication skills. Strong customer service and relationship-building abilities. Ability to manage multiple priorities in a fast-paced environment. Strong problem-solving and critical-thinking skills. Self-motivated with the ability to work independently. Basic computer proficiency, including Microsoft Office and Outlook. Ability to pass all required background screenings. Required Licenses & Transportation Valid driver's license. Clean driving record. Current auto insurance. Reliable transportation for business use. Vehicle provided or monthly vehicle allowance available. Preferred Qualifications Two or more years of experience in home care, home health, hospice, senior services, or healthcare coordination. Previous CNA, LPN, RN, MA, or caregiver experience. Experience providing hands-on caregiver training. Knowledge of community resources available to seniors and their families. Travel Requirements This position requires frequent travel throughout: Illinois Cook County Will County Kankakee County Indiana Lake County Porter County LaPorte County Jasper County Candidates should be comfortable spending significant time in the field visiting clients, caregivers, referral partners, and healthcare facilities.
