Hospice Crisis Care LPN Day Shift

JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

HOURS:

Monday•Friday (3) 12-Hour Shifts 7AM-7PM and Every Other Weekend

Years of Experience Qualifications:

Graduate of an accredited practical nurse or a vocational nursing program Three years of nursing experience. Provide direct patient care as defined in State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
