Responsibilities Monitor, record, and report symptoms or changes in patient's conditions. Maintain accurate patient health records and report any discrepancies to the supervising nurse or physician. Administer medications and treatments to patients, observing reactions or side effects. Assist with daily living activities such as bathing, dressing, and eating. Follow a specific care plan for patients and report on completed tasks. Assist in the implementation of nursing care plans and provide feedback to improve patient outcomes. Educate patients and their families on proper home health care strategies and procedures.
