Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life—empowering patients to receive the care they need right where they feel most comfortable: home. Responsibilities Conduct comprehensive head-to-toe assessments and carry out personalized care plans. Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support. Approach every patient interaction with professionalism and genuine empathy. Guide and empower patients and caregivers with education on disease management, emergency response, and therapeutic techniques. Partner closely with physicians, therapists, and fellow team members to deliver the best possible care. Keep accurate, timely documentation of all care activities in line with agency and state standards. Uphold the highest standards of safety, infection control, and patient privacy. Shift Information Shifts range from 4 to 12 hours, tailored to patient needs and nurse availability. Flexible scheduling for day, evening, and weekend shifts. Choose from consistent long-term assignments or varied schedules. Qualifications Active LPN license in Indiana or an active compact LPN license. Minimum of [1-2] years of clinical nursing experience (home health and pediatric experience strongly preferred). Experience with G-tube, trach, or vent care is a plus (training provided).
