This is an excellent opportunity for a nurse who enjoys building meaningful relationships with patients and families while providing high-quality, individualized care. Our nurses provide focused, personalized care while working independently in a home-care environment with the support of an experienced clinical leadership team. $30- 37 per hour, based on licensure, experience, credentials, and shift requirements Position Responsibilities The Private Duty Home Care Nurse will provide skilled nursing services according to each client's individualized Plan of Care and physician orders.
Responsibilities may include:
Provide one-on-one skilled nursing care to clients in their homes. Perform ongoing nursing assessments and recognize changes in the client's condition. Monitor vital signs and other clinically appropriate parameters. Administer medications and treatments according to physician orders and the Plan of Care. Provide respiratory care, tracheostomy care, suctioning, oxygen therapy, and other skilled interventions as ordered and within the nurse's scope of practice. Provide enteral feedings and medication administration as ordered. Monitor for changes in condition and promptly communicate significant findings to the appropriate healthcare provider and clinical team. Follow established emergency procedures and respond appropriately to changes in the client's condition. Provide wound care and other skilled nursing interventions as ordered. Maintain accurate, timely, and complete clinical documentation. Communicate effectively with clients, families, physicians, therapists, case managers, and BrightStar Care clinical leadership. Provide education and support to family members and caregivers within the nurse's scope of practice and agency policies. Maintain patient safety and follow infection prevention and control practices. Participate in care-plan updates and communicate changes in the client's condition to appropriate members of the healthcare team. Maintain HIPAA compliance and protect client privacy at all times. Provide care in accordance with BrightStar Care policies, applicable laws and regulations, and professional standards of practice. Qualifications Required Current, unrestricted Indiana LPN license. Current CPR/BLS certification. Successful completion of required background screening and employment requirements. Ability to meet agency health, competency, and credentialing requirements. Reliable transportation and ability to travel to the client's home. Strong assessment, communication, and clinical documentation skills. Ability to work independently while maintaining communication with the agency's clinical team. Preferred Previous private duty nursing or home health experience. Experience providing one-on-one skilled nursing care. Experience with tracheostomy care, suctioning, oxygen therapy, enteral feeding, or complex medication regimens. ICU, step-down, respiratory, pediatric, long-term acute-care, or complex-care nursing experience. Experience caring for medically complex clients. Additional specialty or competency-based clinical training. Pediatric nursing experience, if applicable to assigned clients.
