PRN Home Health Field Nurse (RN/LPN) - Michigan City, IN About Us Home Care Services of Northwest Indiana is dedicated to helping individuals live safely and independently in their own homes. Our team of compassionate clinicians delivers skilled, personalized care that supports patients through recovery, chronic illness management, and everyday health needs — all in the comfort of home. Position Summary We are seeking a PRN Field Nurse (RN or LPN) to provide skilled home health visits to patients in and around Michigan City, Indiana. This role offers flexible, as-needed scheduling and is ideal for a nurse who values autonomy, meaningful patient relationships, and the ability to make a direct impact in the community. Key Responsibilities Conduct in-home nursing visits per the physician-directed plan of care Perform skilled nursing tasks including wound care, medication management, injections, and vital sign monitoring Assess patient condition and communicate changes to the care team and physician Educate patients and family members/caregivers on disease management, medications, and home safety Maintain accurate, timely documentation in compliance with agency and regulatory standards Coordinate care with interdisciplinary team members as needed Qualifications Active, unencumbered RN or LPN license in the state of Indiana Current CPR certification Reliable transportation and willingness to travel to patient homes in the Michigan City area Strong clinical judgment, communication, and time-management skills Prior home health experience preferred but not required What We Offer Flexible, PRN scheduling to fit your availability Competitive per-visit pay Mileage reimbursement Supportive clinical team and ongoing training Opportunity to build long-term patient relationships in a rewarding, community-based setting
