In this vital role, you will provide compassionate, high-quality nursing services across various settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans. Administer medications and injections accurately following physician orders while adhering to strict safety protocols and HIPAA regulations. Assist with medical procedures such as IV infusion, catheterization, wound suturing, and specimen collection while maintaining aseptic techniques. Manage patient documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring precise medical records and compliance with healthcare standards. Provide specialized care for patients with complex needs including Alzheimer's disease, dementia, memory care, hospice requirements, and acute pain management. Support emergency response efforts by managing airway management, ventilator use, and responding effectively to urgent medical situations in settings like Level I Trauma Centers or ICU environments. Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to promote optimal recovery and community reintegration. Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in diverse healthcare settings such as hospitals (including ICU or emergency room), nursing homes, assisted living facilities, or home care environments. Demonstrated proficiency in medical terminology, anatomy and physiology knowledge, ICD-9/ICD-10 coding, and medical documentation practices. Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate patient data management. Skilled in performing venipuncture, phlebotomy, medication administration via injections or IV insertion, and sterile processing techniques. Strong understanding of infection control protocols and HIPAA compliance to ensure patient privacy and safety. Ability to work effectively with individuals of all ages—including pediatrics and neonatal patients—and those with disabilities or complex health conditions such as infectious disease or spinal injuries.
