Apply Referral Coordinator LPN/MA Leavenworth, KS Apply Description AllWays Community Health Center is currently seeking a Referral Coordinator to join our team in Leavenworth, KS. Position Summary:
The Referral Coordinator is an integral part of the ACHC's clinical team who is responsible for coordinating all patient referrals at the request of the providers. He/she will schedule all appointments necessary, coordinating with the patient and the outside Providers. Will notify patient of said appointment and give instructions if needed. Will enter all referrals into the eCW system, will track and record reports as they are received. The Referral Coordinator is also responsible for tracking and coordinating ancillary services, as well as communicating with the proper lab, facility and/or patient to facilitate/follow up on those services. The Referral Coordinator will also be cross trained to provide coverage for the clinic nurses if needed due to staffing issues/shortages. Requirements 1. Works in harmony with others, exhibits a positive attitude and commitment to the health center mission. 2. Represent the clinic to the community in a consistently positive and effective manner. 3. Demonstrates flexibility in work roles and schedules to meet the changing needs of the center. 4. Track and coordinate all patient referrals and testing/imaging orders from the providers. 5. Schedule patients for referral appointments and testing/imaging appointments as needed. 6. Complete prior authorizations as needed for referral and testing/imaging in a timely manner. 7. Notify patients of all upcoming appointments and instructions, if needed. 8. Follow up with patient/facility if report is not received after appointment date. 9. Fax clinical information or medical records to outside facilities as indicated. 10. Maintains awareness of new information applicable to job, i.e. policies, meeting minutes, procedures, memos, and protocols. 11. Consistent attendance and participation in meetings that you have been designated to attend. 12. Maintains professional appearance; follows established dress code. 13. Promptly informs Supervisor of real or potential clinic problems. 14. Maintains confidentiality of patients and staff. 15. Actively pursues continuing education to improve job skills or performance. 16. Maintains CPR/BLS competency. 17. Will be cross trained to provide coverage for the clinic nurses. 18. Attend and successfully complete all required training programs; participate in on-going conference calls, webinars, and other professional development opportunities. 19. Perform other related work as assigned or requested.
Skills:
1. Preferred medical office experience, knowledge of medical terminology and
HIPAA. 2.
Ability to recognize and assist with stat or urgent referrals. 3. Ability to work with clinical staff to handle pressure from multiple patients, staff, and providers. 4. Commitment to the mission of ACHC. 5. Exhibits the ability to manage time well and work under stressful conditions with an even temperament. 6. Displays ability to establish and maintain harmonious working relationships with other employees and the public. 7. Demonstrates ability to understand and follow oral and written instructions. 8. Ability to learn about patients and their challenges. 9. Must have the ability to address sensitive issues in a confidential manner. 10. Knowledgeable in motivational interviewing. 11. Ability to interact with patients and/or family members at their level of understanding. 12. Ability to work as a team member. 13. Ability to work independently, self-motivated, and able to take responsibility for their own performance. 14. Excellent verbal, listening and written communication skills. 15.
