This role is ideal for candidates with experience in hospice nursing, home health, palliative care, end-of-life care, admissions, after-hours triage, symptom management, and compassionate patient-centered care. As a nonprofit, faith-based hospice organization, our focus is providing compassionate end-of-life care, hospice services, palliative support, and holistic care that meets the physical, emotional, and spiritual needs of patients and families. As our On-Call Hospice LPN, you will provide after-hours nursing support, triage urgent patient needs, respond to symptom management concerns, coordinate crisis care when needed, and ensure patients and families receive compassionate care and peace of mind during critical moments. Great Benefits Include Medical, dental, and vision insurance available after 30 days Paid time off (PTO) Paid holidays Retirement plan / 401(k) match Mileage reimbursement Continuing education support Professional development opportunities Supportive team culture Key Responsibilities Provide after-hours on-call nursing support for hospice patients and families Respond to urgent patient needs, symptom management concerns, and crisis situations Perform telephone triage and determine when in-person visits are necessary Complete evening, overnight, and after-hours patient visits as needed Provide pain and symptom management interventions with compassion and urgency Coordinate care with physicians, families, facilities, and the hospice interdisciplinary team Support hospice admissions when needed during after-hours coverage Document patient care accurately and timely in MatrixCare EMR Provide education, reassurance, and guidance to patients, families, and caregivers Ensure compliance with hospice regulations and standards of care What Success Looks Like Success in this role means: Timely response to urgent patient and family needs Compassionate symptom management and crisis intervention Strong clinical judgment and independent decision-making Excellent communication with families, facilities, and providers Accurate and timely documentation Peace of mind and comfort for patients and families during after-hours care Qualifications Preferred Experience Experience in hospice nursing, home health, palliative care, oncology, med-surg, geriatrics, or end-of-life care preferred Candidates with hospice, home care, after-hours triage, admissions nursing, or skilled nursing experience are strongly encouraged to apply Experience working independently in field-based nursing roles is highly valued Experience using MatrixCare EMR or hospice documentation systems preferred Required Education & Licensure Active LPN license in the state of Kansas required Graduate of an accredited nursing program required Required Skills Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Compassionate, calm, and professional bedside manner Ability to manage urgent situations independently and confidently Strong documentation and organizational skills Commitment to exceptional patient-centered care Requirements Valid driver's license required Reliable transportation for patient visits Ability to pass background check Our Mission At Catholic Community Health, we share Christ's love by providing compassionate, high-quality healthcare to seniors and those nearing the end of life in an environment where every person is treated with dignity, regardless of faith or ability to pay.
