The LPN plays a critical role in delivering quality care to and support for both new and existing residents and the subsequent development of the care or service plan. This role involves direct resident care, coordination with healthcare professionals, and promoting an environment of respect, dignity, and well-being. Responsibilities Administer medications and injections as prescribed by physicians. Provide hands-on nursing care in accordance with resident care plans and regulatory standards. Monitor and document residents' conditions, reporting changes to RN, physician, or appropriate team members. Assist with pain management strategies and provide support for patients dealing with various ailments. Assist with development and revision of individualized care plans. Support residents with daily living activities as needed. Foster a safe, clean, and comforting environment for residents. Supervise and guide CNA's and other support staff. Communicate effectively with residents, families, and medical professionals. Participate in care team meetings and quality improvement initiatives. Ensure compliance with state, local, and federal regulations, including HIPPA Provide education to patients and families regarding health management and family planning. Respond to emergency situations promptly and effectively within the scope of practice following community protocols. Requirements Current LPN license in good standing. Minimum of 1 year nursing experience, preferably in senior care, memory care, or a similar setting. Proficiency in medication administration and experience administering injections. Compassionate, patient-centered approach to care. Strong understanding of vital signs monitoring and documentation practices. Ability to work collaboratively within a healthcare team while maintaining excellent communication skills. Knowledge of dementia and Alzheimer's is a plus.
