Job Description

Licensed Practical Nurse (LPN) responsible for assisting with patient care, promoting wellness, maintaining accurate documentation, and ensuring quality, compassionate service. Essential Duties and Responsibilities Room patients and obtain vital signs. Document chief complaint, medical history, allergies, and medications. Administer medications, immunizations, and injections as ordered. Perform EKGs, point-of-care testing, and specimen collection Process medication refills and prior authorizations Educate patients regarding medications, treatment plans, and preventive care. Maintain accurate documentation in the electronic health record (EHR). Respond appropriately to medical emergencies within scope of practice Qualifications Current Kentucky Licensed Practical Nurse (LPN) license in good standing. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to work independently and as part of a team. Proficiency with electronic health records preferred. Preferred Qualifications One year of primary care, family medicine, or outpatient clinic experience. Experience with immunizations, chronic disease management, and patient education.
