Job Description

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to patients across diverse settings, including long-term care facilities, home health, hospitals, and outpatient clinics. Your expertise will support patient well-being through comprehensive assessments, medication administration, and patient education. The ideal candidate will demonstrate strong clinical skills, a commitment to patient-centered care, and proficiency with electronic health record systems. Medplus staffing offers a variety of LPN positions: PRN, ongoing, temp to hire and contracts. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Records) systems Administer medications via various routes including injections, IV infusions, and catheterizations while adhering to safety protocols and HIPAA regulations. Manage complex care needs such as Alzheimer's disease, dementia care, memory care, hospice support, and acute pain management. Assist with diagnostic evaluations including specimen collection/processing, phlebotomy, and medical imaging procedures. Provide specialized care for patients with infectious diseases, respiratory issues requiring ventilator management, or those in critical care environments such as ICU or Level I trauma centers. Support discharge planning and coordinate with multidisciplinary teams to ensure seamless patient transitions from hospital to home or long-term care settings. Maintain accurate medical documentation of patient care activities and treatment plans in compliance with healthcare standards. Educate patients and their families on health coaching topics such as medication management, nutrition, and chronic disease management. Ensure infection control practices are followed diligently to prevent cross-contamination in all clinical environments. Collaborate effectively with physicians, case managers, and other healthcare professionals to optimize patient outcomes. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Atleast 6 months verifiable experience as a LPN Proficiency with EMR/EHR systems Knowledge of medical terminology, anatomy and physiology principles relevant to patient assessment and treatment. Experience with IV infusion therapy, catheterization procedures, sterile techniques, and basic life support certification. required certifications for unit such as:
BLS, ACLS, PALS, NIH
