Job Description

he Population Health Coordinator assists in organization-wide efforts for Population Health initiatives and tasks that drive success of the Population Health program within the health system. The Population Health Coordinator will provide oversight to the scheduling process and prioritization of wellness visits, care gap closures, and point of care solutions for providers including daily huddles. The coordinator will work with clinic staff on EMR assistance, PCP attribution improvements, and clinic workflows to ensure overall success of the program. The coordinator will provide updates on performance reporting to CEO, clinic staff, and other leadership team to improve outcomes. The coordinator will understand value-based arrangements and work with the managed care team to distribute reporting with a systematic approach to improve healthcare outcomes for patients including but not limited to care gaps, diagnosis coding gaps, hospital & ER follow up, pharmacy initiatives, & chronic care management.
