Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care in the nursing home setting. As an LPN, you will work under the supervision of registered nurses and physicians, ensuring that patients receive the best possible care in accordance with established standards and protocols. Experience in PCC or other EMR preferred. Day shift, rotating shift, 3/2 schedule. Responsibilities Deliver direct patient care, including administering medications and treatments as prescribed. Monitor and document patient vital signs and medical history accurately. Assist in the development and implementation of individualized care plans. Collaborate with healthcare team members to ensure comprehensive patient care. Provide support to patients and their families, addressing any concerns or questions. Maintain a clean and safe environment for patients, adhering to infection control standards. Participate in ongoing training and professional development activities. Ensure compliance with all relevant regulations, including ICD-9 coding standards. Requirements Current LPN license in good standing. Previous experience in an long term care setting preferred. Familiarity with electronic medical records systems, particularly PCC. Strong understanding of medical documentation practices and standards. Ability to work effectively in a fast-paced environment while maintaining attention to detail. Excellent communication and interpersonal skills to foster positive relationships with patients and staff. Commitment to providing compassionate care to all patients.
