$65,000- $79,000 The posted range represents the full earning potential for the role over time, not the typical starting or hiring rate. Starting pay is determined based on factors such as relevant experience, skills, internal equity, and market alignment. Company Overview Duffy Health Center, a non-profit located in Hyannis, MA, provides compassionate and skilled integrated healthcare for persons with complex health and social needs on Cape Cod. We envision a Cape Cod where all persons have access to quality health care, safe and stable housing, and lives filled with connection and purpose. Duffy Health Center is a Federally Designated Healthcare for the Homeless Program. This requires that most of the patients we serve are experiencing or have recently experienced homelessness or some other form of housing insecurity. Staff choose to work here because they believe deeply in the mission, and some staff have relevant lived experience themselves. Position Summary Key Responsibilities The statements contained in this job description reflect general details, as necessary, to describe the principal functions of this job, the level of knowledge and skill typically required, and the scope of responsibility. Individuals may perform other duties, as assigned, including work in other functional areas to cover absences or provide relief, to equalize peak work periods or otherwise to balance the workload. Engage with patients regarding appointments, both in person and virtually via telehealth. Anticipate and complete appropriate PreVisit Planning activities for individual visit types Obtain and document vital signs, point of care testing and EKGs according to evidenced based standards. Administer vaccines and therapeutic injections as ordered, documenting results, Triage patients and clients without hesitation and prioritize their needs. Escalate clinical or behavioral health issues when appropriate to the covering Provider Collaborate with other health care team members to optimize clinical outcomes and patient treatment plan. Provide education to patients in person, via telephone or via telehealth on chronic disease management, preventative care recommendations and health care navigation. Identify knowledge gaps. Use motivational interviewing techniques to support patients in achieving their health and wellness goals. Connect patients and clients to community resources or behavioral health options when needed. Build relationships with community partners, such as pharmacies, inpatient/outpatient facility personnel, and community resources. Be flexible and adaptive within the role, as the position can be extremely fluid at times and requires a great deal of teamwork. Lead by example and support practice changes and processes in the medical department. Participate in interdisciplinary team meetings including but not limited to Huddle, Nursing Team meetings, and Medical Floor meetingsPopulation Served Unhoused Housing Insecure SUD/BH Clients Education & Experience:
(Required & Preferred) A candidate for this position must have a Licensed Practical Nursing license; and at least one year of experience in a clinical setting similar to the Duffy Health Center. Accredited completion of a practical nurse education program in the state of Massachusetts or approved by the Massachusetts Board of Nurses. Applicant must also possess a good moral character and achieve a passing score on theNCLEX-PN
if right out of education program. Experience with Epic electronic health record a plus.Skills & Competencies:
A candidate for this position should have the following: Nursing theory and practice as relates to the LPN level Rules, regulations, and policies of the health care facility Trauma informed care within the context of homeless health Motivational interviewing and Harm Reduction model in the context of health care. Primary care, mental health, and substance abuse, and/or community health care environment Medical office practices, procedures, and terminology Disease processes and medications Electronic Medical Records systems HIPAA regulations : Compassionate patient care, including thorough health screening and testing of vital statistics (blood pressure, pulse, height, weight, etc.) Computer use and Windows software Written, verbal and interpersonal communication skills Interacting effectively and appropriately with homeless and at-risk individuals Protocols for handling emergency/crisis situations Examining and treating patients within the scope of licensure Learn and to be mentored Manage multiple priorities simultaneously in a timely manner Work effectively under a high volume of patient interaction Interact effectively and appropriately with a diverse population, including homeless and patients with substance use disorder Accurately collect and record information, prepare reports and information Work in a fast-paced environment with a complex patient population Establish and maintain strong, effective, and therapeutic relationships with patients Commit to the philosophy and mission of Duffy Health Center which serves the homeless and at-risk population, and to the team-based approach as part of PCMH The nature of work means that errors can be easily detected, usually by the employee. Consequences of errors missed deadlines, or poor judgment may result in minor confusion, involving minimal time and expense for correction, such as taking blood pressure, data entry or failure to follow established procedures. Standardized practices, procedures, or general instructions govern the work which in some cases, may require additional interpretation. Judgment is needed to locate, select, and apply the most pertinent practice, procedure, regulation, or guideline. The work consists of the practical application of a variety of concepts, practices and specialized techniques relating to a professional or technical field. Assignments typically involve evaluation and interpretation of factors, conditions, or unusual circumstances; inspecting, testing, or evaluating compliance with established standards or criteria; gathering, analyzing and evaluating facts or data using specialized fact-finding techniques; or determining the methods to accomplish the work. Under general direction, the employee plans and conducts the regular work in accordance with standard practices and previous training, with substantial responsibility for determining the sequence and timing of action and substantial independence in planning and organizing the work activities, including determining the work methods. The employee is expected to solve, through experienced judgment, most problems of detail or unusual situations by adapting methods or interpreting instructions to resolve the problem. Instructions for new assignments or special projects usually consist of statements of desired objectives, deadlines, and priorities. Technical and policy problems or changes in procedures are discussed with supervisor, but ordinarily the employee plans the work, lays it out and carries it through to completion independently. Work is generally reviewed only for technical adequacy, appropriateness of actions or decisions, and conformance with policy or other requirements; the methods used in arriving at the end result are not usually reviewed in detail. Relationships with co-workers and the general patient population involving frequent explanation, discussion or interpretation of practices, procedures, regulations, or guidelines to render service, plan, or coordinate work efforts, or resolve operating problems. Other regular contacts are with patient/service recipients and employees of outside organizations such as vendors, banks and/or developers/contractors. More than ordinary courtesy, tact and diplomacy may be required to resolve complaints or deal with hostile, uncooperative, or uninformed persons.Work Environment/Physical Requirements:
Working conditions involve exposure to intermittent machine or related noise or a combination of unpleasant elements such as odors, chemical fumes, dust, smoke, heat, cold, oil, dirt, or grease. Work may also involve occasional mental stress, such as completing several unrelated tasks within a short period of time. Work requires some agility and physical strength, standing or walking most of the work period, or helping clients or patients move. Occasionally, work may require lifting heavy objects and carrying them (up to 10 lbs.). Duties may involve close hand and eye coordination and physical dexterity. Manipulation and motor control under conditions which may require extreme accuracy may be critical. The manual skills required are comparable to those which might be needed in conducting laboratory tests, performing microscopic experiments, or administering injections. Duties regularly present the potential risk of injuries from improper exposure which could result in loss of time from work. Examples of injury include illness from exposure to communicable diseases or direct exposure to bodily fluids. Special safety precautions, training, or protective clothing such as gowns, coats, gloves, glasses may be required. Has access to confidential information obtained during performance of regular position responsibilities, where the effect of any disclosure would be negligible or where the full significance of the overall confidential matter would not be apparent in the work performed. The Licensed Practical Nurse (LPN) is responsible for observing patients, charting and reporting changes in conditions, measuring and recording patient vital signs and providing basic patient care or treatment such as medication administration. The LPN works within an integrated health care team collaborating with and supporting the Psychiatric Providers. The LPN will have effective and therapeutic communication skills while demonstrating compassion through positive relationship development. A candidate for this position must have an Associate's Degree, Bachelor's preferred, valid LPN license and at least one (1) year of experience, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.
