What You'll Do:
Monthly Visits:
Conduct routine home visits, document ADLs, and submit complete clinical notes within 24 hours.
Care Planning:
Use your expertise to develop, update, and individualize member Plans of Care.
Assessments & Compliance:
Handle semi-annual health reports, annual MDS reviews (including psychosocial/fall risks), and coordinate with PCPs.
MDT Collaboration:
Call into monthly multidisciplinary team meetings to present quick case summaries.
What We're Looking For:
Active LPN License in Massachusetts (Required) Home health or community-based nursing experience (Highly preferred—your background means you can hit the ground running!) Reliable transportation for local in-home visits Sharp documentation skills and strong time management Experience with EMR networks is a huge plus! Ready to Maximize Your Schedule?
