Roxbury, Hyde Park, Roslindale, Dedham, Norwood, Brighton, Brookline, Quincy, Mattapan, Newton, Allston, Cambridge, Brockton, Randolph, Milton, Lynn, Chelsea, East/South Boston, Medford, Somerville (all territories are not expected to be covered per position) Home Care LPN Responsibilities Assess the patient's condition during every visit and document your observations Perform different tasks including vital signs and medication review Administer medication as prescribed by the patient's Physicians Perform wound care and assess the progress of healing Educate patients and their families on proper home health care strategies and procedures Coordinate with Physicians and anyone else involved with the patient's care plan Make recommendations for devices/tools that might improve the patient's quality of life Listen to the patient and respond to concerns or requests Able to complete clinical documentation within 24 hours of Patient contact. Qualifications Must have a criminal background check State LPN/LVN license required 1 year of experience in in-home care is preferred, but we can train the right candidate Excellent communication and time-management skills Compassionate and friendly demeanor Ability to travel to multiple patients per day Weekends shift

Pay:
