Long-Term Care / Skilled Nursing Facility Job Summary "Navitas Healthcare, LLC" is seeking an experienced and compassionate Licensed Practical Nurse (LPN) for a 13-week contract opportunity within a long-term care and skilled nursing facility setting in Falmouth, Massachusetts. The ideal candidate will have strong long-term care experience and the ability to provide high-quality resident-centered care in a fast-paced healthcare environment. This role involves medication administration, direct resident care, clinical documentation, and supervising support staff while collaborating with interdisciplinary healthcare teams. Key Responsibilities Provide direct nursing care in accordance with physician orders and individualized care plans Administer medications, treatments, and therapeutic procedures safely and accurately Supervise nursing assistants and support staff during assigned shifts Monitor resident conditions and report changes to physicians and nursing leadership Maintain accurate documentation including nursing notes, physician orders, and resident records Assist with admissions, transfers, and discharges Perform treatments including wound care, catheter care, suctioning, tube feedings, dressing changes, and specimen collection Monitor and document vital signs and resident assessments Educate residents and families regarding care plans and treatment protocols Participate in interdisciplinary care planning and support quality care initiatives Ensure compliance with facility policies, patient safety standards, and resident rights Maintain confidentiality of resident health information Requirements Active Massachusetts LPN License required Graduate of an accredited Nursing Program or approved LPN Program required Previous long-term care or skilled nursing experience preferred Strong leadership, communication, and documentation skills Ability to work effectively in a fast-paced healthcare environment Ability to meet physical job requirements, including lifting up to 50 pounds English proficiency required (Read, Write, Speak) Education Nursing Degree from an accredited college/university OR approved LPN program required Certifications Massachusetts LPN License required
