Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hathorne Hill Rehabilitation and Healthcare Center in Danvers, Massachusetts. As an LPN, you will play a vital role in providing high-quality care to our residents, working under the supervision of our RN Supervisor/ADNS. Administers medication and treatments according to physician's orders Assists with ordering medications Transcribes, documents, and implements physician's orders Collects data to assist in nursing assessments during each shift Gives daily shift report to CNAs assigned to the unit Reviews written daily assignments for each resident on assigned unit Assures assignments are completed before the end of the shift Provides care to residents, including making rounds at least once every two hours Collaborates with Care Plan Coordinator to develop and maintain nursing care plans Attends and participates in interdisciplinary resident care conferences Assists attending physician with rounds and special procedures as needed Participates in yearly written evaluations of nursing assistants Assists Staff Development with orientation and on-the-job training for new nursing personnel Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants Notifies nursing supervisor, resident's physician, and family of a change in the resident's condition or an accident/incident involving the resident Assumes any duties assigned by the RN Supervisor/ADNS to provide optimal quality resident careRequirements:
Licensed Practical Nurse licensed in the State of Connecticut Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred CPR certified Ability to read, write, and comprehend English; ability to follow oral and written instructions Fluent knowledge of the MS Office suite and other office equipment Ability to work hours as scheduled based on the requirements of the position assignment Physically able to move intermittently throughout the day, proper body mechanics required Ability to lift a minimum of 50 lbs.Behavioral Competencies:
