11p-7a shift- per diem includes one weekend a month Summary We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our dedicated team at Alliance Health and Human Services. In this role, you will provide essential patient care in our nursing facility, ensuring that elders receive the compassionate support they need to thrive. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor patient health status and report any changes to the healthcare team. Supervise nursing assistants and monitor care provided. Assist in the development of individualized care plans for patients. Maintain accurate patient records and documentation in compliance with regulations. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Requirements Current LPN/RN license in the state of practice. Experience in senior care nursing preferred. Strong case management skills to coordinate patient services effectively.
