for Home Health Visits!

• HomeCare Hands is hiring per diem Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide home health medication management visits and wellness checks for clients living independently in the community.

This role functions as a visiting nurse position, focusing primarily on medication management, brief wellness assessments, and care coordination . Most visits take 30 minutes or less and typically occur once per week per client. Visits are scheduled geographically whenever possible to minimize travel time. Flexible per diem scheduling allows you to build a schedule that fits your availability,. Opportunities to pick up additional visits if desired Competitive per-visit compensation based on visit complexity paid bi-weekly Nursing Visit Responsibilities Administer prescribed medications or organize medications into a weekly pill planner Complete a brief client wellness assessment Provide medication-related case management & communication Document visits according to agency standards Support clients in maintaining safe and independent living at home
