We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Nemasket Rehabilitation and Healthcare Center in our subacute unit. As an LPN, you will be responsible for providing high-quality patient care to our residents, ensuring their overall well-being and satisfaction. Responsibilities:
Administers medication and treatments according to physician's orders Assures assignments are completed before the end of the shift Collaborates with Care Plan Coordinator to develop and maintain nursing care plans Conducts daily rounds on assigned residents Documents accurately resident's strengths, weaknesses, and the steps needed to improve performance Gives daily shift report to CNAs assigned to the unit Participates in interdisciplinary resident care conferences Participates in the yearly written evaluations of nursing assistants Assists Staff Development with orientation and on-the-job training for new nursing personnel Assists with special procedures and rounds as needed Notifies nursing supervisor, resident's physician, and family of any changes in the resident's condition or accidents/incidents involving the resident
Requirements:
Licensed Practical Nurse (LPN) licensed in the State of Massachusetts Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred CPR certified Ability to read, write, and comprehend English; ability to follow oral and written instructions Ability to work hours as scheduled based on the requirements of the position assignment Must be able to move intermittently throughout the day, proper body mechanics required. Ability to perform physically demanding work involving frequent bending, stooping, turning, stretching, and reaching above the shoulders are involved Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately to assure that the requirements of this position can be fully met Must be able to lift a minimum of 50 lbs.
