Per Diem LPN Department:
Residential Location:
Middleton, MA

All Shifts Available including 9am-3pm, 3am-11pm, 11pm-7am
Location:
Danvers, Middleton, Burlington, Topsfield Pay:
$42 /hr. The Per Diem LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence.
Primary Responsibilities:
Understand the clinical and social needs of the individuals' receiving services. Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care. Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed. Present information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and /or email correspondences. Responsible to assist at physician's appointments, community trips, and other activities as assigned. Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.
Requirements:
