AlphaCare Home Health is a Medicare-certified, CHAP-accredited agency serving southeastern Massachusetts. We have separate LPN openings in New Bedford , Fall River , Attleboro , and Taunton , at every schedule — full-time, part-time, and per diem. You pick your territory. Choose New Bedford, Fall River, Attleboro, or Taunton, and that is where your visits stay. Each nurse covers a single community and the immediate area around it, so your drives are short and close to home. Pick the area and the schedule that work for you, and we will build around it. Choose your schedule Full-time — salaried or 40 visits per week, with full benefits Part-time — a set weekly commitment, with benefit eligibility at 20+ visits per week Per diem — take the visits that fit your week, no minimum You'll be paired with an RN Case Manager You will never be figuring out a case on your own. Every LPN is paired with the RN Case Manager who owns that patient's plan of care — your first call for questions, changes in condition, or anything you find in the home that does not look right. Behind that pairing is the rest of the team: clinical managers available for real-time support, a Director of Nursing overseeing clinical practice, and a clinical compliance and QA team that reviews documentation so issues get caught and corrected before they become your problem. New hires receive paid training and orientation to our clinical processes and documentation system — you are compensated for that time, not expected to learn on your own hours — plus ongoing support for as long as you need it. What you'll do Provide skilled nursing visits within the LPN scope of practice, following the plan of care established by the RN Case Manager Administer medications and treatments, perform wound care, and complete ordered procedures Observe and record patient condition, and report changes promptly to the RN Case Manager and physician as directed Complete accurate, timely visit documentation in Netsmart DeVero Reinforce patient and caregiver teaching on medications, symptom management, and disease progression Coordinate with therapy, social work, and home health aide staff on the patient's care team What you need Graduate of an accredited practical nursing program Current Massachusetts LPN license Minimum 1 year of nursing experience Current BLS certification Valid driver's license, reliable vehicle, and current auto insurance Home health experience strongly preferred Behavioral health or psychiatric nursing experience a plus — many of our patients have co-occurring behavioral health needs Benefits Paid time off Paid holidays Mileage reimbursement Health and dental insurance Short-term and long-term disability 401(k) with company match Paid training and license renewal reimbursement Benefit eligibility varies by schedule — specify which benefits apply to full-time, part-time, and per diem.
