Job Overview We are seeking a dedicated and compassionate Per diem Licensed Practical Nurse (LPN) to join our healthcare team. This role offers flexible scheduling to support various clinical settings, including long-term care facilities, hospitals, assisted living communities, and home care environments. The LPN will provide high-quality patient care by performing assessments, administering medications, and supporting treatment plans under the supervision of registered nurses and physicians. The ideal candidate will possess a strong foundation in patient care, excellent communication skills, and the ability to adapt to diverse clinical situations. This position emphasizes delivering safe, effective, and compassionate nursing services while maintaining compliance with healthcare regulations and confidentiality standards. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Support patients with activities of daily living, including mobility assistance, tube feeding, wound care, and basic life support procedures. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration requiring urgent intervention or triage. Maintain detailed and accurate medical documentation in compliance with HIPAA regulations and long-term care standards. Assist with diagnostic evaluations such as specimen collection, laboratory processing, or medical imaging preparation. Collaborate effectively with multidisciplinary teams to coordinate patient care plans for individuals with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Ensure infection control practices are rigorously followed to prevent cross-contamination in all clinical activities. Educate patients and caregivers on health management strategies including medication administration, nutrition, and discharge planning. Support hospice or palliative care services by providing compassionate end-of-life comfort measures.
