Admissions Licensed Practical Nurse Qualifications:

The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing.

Job Responsibilities:

The Licensed Practical Nurse will:

• Maintain consistent and ongoing communication with the nursing team regarding scheduling of pre-admission assessments, initial assessments, PCP order form status updates, PA submissions and placements.
• Conduct a member assessment to determine eligibility status based on MassHealth regulations as it pertains to Adult Foster Care services (AFC).
• Conduct an RN assessment on the potential caregiver to determine if they understand the member's condition and whether they are suitable to provide the necessary care to the potential member.
• Select, train, evaluate, and supervise AFC caregivers in conjunction with the care manager.
• Complete nursing assessments and coordinate all other applicable clinical assessments.
• Once completed, sign the designated screening tool as required under 130 CMR 408.416(C).
• Prepare, review and submit Primary Care Physician (PCP) Order Forms to the PCP once pre-admission assessment is completed and potential member appears to qualify for AFC services
• Send fax and follow up consistently with doctors/clinics to ensure they respond in a timely manner
• Follow up with all relevant specialists to receive pertinent paperwork that may be needed to support the case that the potential member needs AFC Services.
• Submit all relevant paperwork to the state for prior authorization approval for AFC services as soon as PCP and/or specialist paperwork is received.
• Persistent checks into the prior authorization portal for any status changes regarding paperwork submitted for approval.
• In a timely manner, bring services to members and caregivers as soon as possible
• Schedule and conduct placements of member/caregiver as soon as possible- once prior authorization approval has been received through the portal.
• Develop and sign the member's AFC interim plan of care within 5 days of placing the member into active AFC services.
• Develop, sign and acquire the signatures of the member, and the assigned care manager within 30 days of placing the member into active AFC services, the member's AFC initial plan of care.
• Conduct an orientation for each caregiver before the caregiver begins personal care of a new member
• Train caregiver on how to complete daily logs of care/journals.
• Complete a nursing progress note corresponding with each on-site visit or encounter, or more often as the member's condition warrants.
• Report changes in the health status of any member to the member's physician.

Further, the AFC Registered Nurse will: Attend agency staff meetings on a weekly basis or as required by the AFC Program Director. Complete a self-evaluation on a yearly basis Participate in regularly scheduled supervisions with assigned manager Participate in relevant training as assigned by manager and/or program director or other supervisor, as needed. Attend supervisory team or individual meetings as required by the AFC Program Director. Have reliable transportation and be able to travel without restriction to qualified settings within the agency's covered area. Perform all other duties as assigned by the AFC Program Director.
