Job Description
Position Overview Our Adult Day Health program is seeking a dedicated LPN to join our team and provide direct nursing care, medication administration, and participant support in a social, community-based environment. This is a stable daytime nursing role focused on preventive care, wellness, and participant engagement. Key Responsibilities Administer medications and treatments per RN direction Record vitals and participant observations Assist with wound care and chronic disease management Maintain accurate, timely nursing documentation Communicate participant status changes to RN Promote participant safety and well-being Safety & fall prevention Qualifications Current Massachusetts LPN license CPR/First Aid certifications Experience with adult or senior populations preferred Strong communication and organizational skills Regulatory Compliance This position meets all Massachusetts
