Job Description

The Nurse Care Coordinator is responsible for providing clinical care management, outreach, and service coordination for individuals living with
HIV/AIDS.
This role works closely with members, healthcare providers, and community organizations to support continuity of care, identify barriers to services, and ensure access to appropriate healthcare and community resources.
  • Minimum Requirements:
