Hours Shift Details:

Combination of day (5:45a-2:15p) and evening (2p-10:30p) hours 40 hours/week. (5

• 8) 14-week contract Deliver comprehensive nursing care to residents, including medication administration, wound care, and vital signs monitoring.

Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility support to promote independence and dignity. Conduct patient assessments and document findings accurately within electronic health record (EHR) systems, ensuring compliance with healthcare regulations. Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans tailored to each resident's needs. Monitor residents for signs of acute or chronic conditions such as dementia, Alzheimer's disease, or infectious diseases; respond promptly to changes in health status. Administer treatments including IV infusions, catheter care, and injections while adhering to aseptic techniques and safety protocols. Educate residents and their families on health management strategies, medication usage, and discharge planning. Maintain strict confidentiality of patient information in accordance with HIPAA regulations and institutional policies. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Prior experience working in long-term care facilities or skilled nursing settings is highly preferred. Demonstrated knowledge of geriatrics, dementia care—including Alzheimer's—and memory support strategies. Proficiency with electronic health records (EHR) systems such as Epic or Cerner; experience with medical documentation is essential. Strong clinical skills including vital signs assessment, medication administration, wound management, and catheterization. Familiarity with long-term care regulations and compliance standards governing nursing home operations. Excellent communication skills for effective caregiver communication and client interaction; compassionate approach toward elderly populations. Ability to handle emergency situations efficiently using CPR/First Aid certification; experience with acute pain management is advantageous. Knowledge of infection control practices and sterile techniques necessary for safe patient care.
