Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves delivering high-quality patient care, supporting medical teams, and ensuring compliance with healthcare regulations. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to enhancing patient well-being through attentive and professional nursing practices. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems. Administer medications and injections accurately following physician orders while adhering to HIPAA and sterile technique protocols. Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging procedures. Manage IV infusions, catheter care, tube feeding, and ventilator support for patients requiring acute or long-term care. Provide specialized care for patients with dementia, Alzheimer's disease, or memory impairments in memory or hospice care settings. Support emergency response efforts by performing first aid, airway management, and CPR in urgent situations. Collaborate with multidisciplinary teams to develop individualized care plans for patients with complex needs such as infectious disease management or trauma cases. Document all patient interactions accurately within EMR/EHR systems while maintaining clinical confidentiality policies. Educate patients and caregivers on medication administration, disease management, and health coaching strategies. Ensure compliance with long-term care regulations and safety standards in skilled nursing facilities or assisted living environments.
