Job Description
Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. In this vital role, you will provide high-quality nursing care to residents in a skilled nursing facility, assisted living community, or similar long-term care environment. Your responsibilities will include assessing patient needs, administering medications, and collaborating with interdisciplinary teams to ensure optimal health outcomes. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to enhancing the quality of life for our residents. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing care and support. Duties Conduct comprehensive patient assessments, including vital signs and physical examinations, to inform individualized care plans. Administer medications and treatments accurately, including injections, IV infusions, and catheter care, following established protocols. Monitor residents for changes in condition, managing acute pain and providing appropriate interventions. Assist with activities of daily living such as mobility support, hygiene, nutrition, and tube feeding. Maintain detailed medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA and long-term care regulations. Collaborate with physicians, therapists, and caregivers to coordinate care plans tailored to residents' needs. Support discharge planning and transition processes for residents moving between levels of care or preparing for hospice services. Participate in infection control practices and uphold sterile techniques to prevent healthcare-associated infections. Provide education and emotional support to residents and their families regarding health conditions and treatment options. Respond promptly to emergencies or urgent situations using first aid skills and airway management techniques.
