Join Our Team! Atrio Home Care— a partnership between Holland Home, Brio and Rest Haven— is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care throughout the Grand Rapids or Holland territory. This role focuses on delivering high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of downtown Grand Rapids or downtown Holland/Lakeshore. Key Responsibilities Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration Follow and support individualized Plans of Care in collaboration with physicians and the care team Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)
