Must be willing to travel. This role involves providing high-quality nursing care to patients in a home care setting, ensuring their health and comfort while promoting independence and well-being. Responsibilities Provide direct nursing care to patients in their homes, following care plans and physician instructions. Monitor patient health and report any changes to the healthcare team. Administer medications, injections, and treatments as prescribed. Maintain accurate and timely documentation of patient care and progress. Educate patients and their families on health management and disease prevention. Collaborate with other healthcare professionals to ensure coordinated care. Requirements Current LPN license valid in the state of Michigan. Previous experience in home health care or a related nursing field preferred. Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Compassionate and patient-centered approach to care. Reliable transportation to travel to patient homes in the Southeast MI Region.
