Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including inpatient units, outpatient clinics, long-term care facilities, and community health environments. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and collaborative care planning. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols while adhering to HIPAA regulations and aseptic techniques. Assist with complex procedures such as IV infusion, catheterization, wound suturing, and sterile processing in accordance with established clinical guidelines. Monitor patients' conditions closely, recognize signs of acute pain or deterioration, and escalate concerns promptly to the healthcare team. Support patient care activities across diverse settings including medical-surgical units, emergency rooms, ICU (Intensive Care Unit), neonatal ventilator management, and hospice care. Collaborate with physicians, case managers, and multidisciplinary teams to develop and implement individualized care plans for patients with conditions such as Alzheimer's disease, dementia, infectious diseases, or developmental disabilities. Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks to ensure accurate medical records and compliance with regulatory standards. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in various healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or home & community care settings. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding for procedures like gastrostomy or spinal taps. Experience with specialized patient populations such as geriatrics, pediatrics—including neonatal ventilator management—and individuals with disabilities or memory impairments. Ability to perform complex skills such as IV insertion/infusion therapy, phlebotomy, tube feeding management, ventilator support, and airway management. Excellent patient assessment skills coupled with critical thinking to identify urgent issues requiring immediate intervention. Knowledge of infection control practices and HIPAA compliance standards to safeguard patient privacy and safety.
