Job Description

Job Overview We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. In this vital role, you will provide compassionate, high-quality nursing care to residents in a long-term care setting, ensuring their safety, comfort, and well-being. The ideal candidate will possess strong clinical skills, a comprehensive understanding of patient assessment, and the ability to work collaboratively with interdisciplinary teams. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical support and assistance. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans. Administer medications and injections accurately following established protocols, including IV infusions and catheter care. Monitor patients for signs of acute pain, infection, or deterioration; respond promptly with appropriate interventions. Assist with complex procedures such as tube feeding, wound suturing, and sterile specimen collection. Maintain detailed and compliant medical documentation within electronic health record (EHR) systems such as Epic or Cerner. Provide specialized dementia and Alzheimer's care, ensuring a safe environment for residents with memory impairments. Support discharge planning and coordinate with physicians, case managers, and family members to ensure seamless transitions of care. Implement infection control procedures adhering to HIPAA regulations and facility standards. Educate residents and their families on health management strategies, including medication administration and nutrition. Participate in ongoing training related to geriatrics, hospice care, acute pain management, and emergency response protocols.
MUST HAVE
  • 2+ Years Experience in LTC or SNF
  • BLS AHA
  • Must have completed at least '1' successful travel assignment as a LPN
  • COVID Vaccine
Active MI License Shift Timings:
Days & Night (12 hrs.) Weekly schedule: 36hr HC1
Pay:
$40.00 - $44.00 per hour
