Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including inpatient units, outpatient clinics, long-term care facilities, and community health environments. Your expertise will help ensure patients receive personalized, efficient, and safe nursing services. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders, including IV infusion and other specialized treatments like botulinum toxin or dermal fillers. Assist with medical procedures such as catheterization, suturing, spinal taps, and specimen collection/processing while maintaining sterile techniques and infection control standards. Monitor patients' conditions closely, manage acute pain, and respond promptly to urgent care needs or triage situations in emergency room or urgent care settings. Collaborate with physicians, case managers, and interdisciplinary teams to develop discharge plans and coordinate post-acute or home & community care services. Maintain detailed medical documentation in compliance with HIPAA regulations and ICD coding standards (ICD-9/10) for accurate billing and record keeping. Support patient education initiatives by providing health coaching on topics like nutrition, dementia care, Alzheimer's care, or managing chronic conditions such as geriatrics or infectious diseases. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics. Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Skilled in patient assessment, medication administration (including injections), IV insertion/maintenance, and catheter care. Ability to work effectively with individuals with disabilities or developmental delays in settings like home & community care or pediatric units. Excellent communication skills coupled with critical thinking abilities to handle triage situations and acute care scenarios confidently.
