Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic home care team. In this vital role, you will provide high-quality nursing care directly in patients' homes, ensuring comfort, safety, and optimal health outcomes. Your expertise will support individuals across a broad spectrum of needs, from chronic conditions to post-acute recovery, fostering independence and improving quality of life. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications via injections, IV infusions, and other routes following strict protocols and HIPAA regulations to ensure patient safety. Manage wound care, catheterization, tube feeding, and sterile procedures with precision and aseptic technique. Educate patients and their families on medication management, disease processes like Alzheimer's or dementia care, and self-care techniques to promote health literacy. Coordinate with multidisciplinary teams for discharge planning, case management, and triage to prioritize urgent needs effectively. Perform specimen collection and processing, phlebotomy, and diagnostic evaluations as required for ongoing patient monitoring. Maintain accurate medical documentation of patient care activities, ensuring compliance with HIPAA standards and ICD coding requirements such as ICD-10 or ICD-9. Experience Proven experience as an LPN in diverse settings such as home care, long-term care facilities, assisted living communities, or hospitals. Critical care experience including ICU or Level I/II trauma center exposure is highly desirable. Familiarity with medical office procedures, EMR/EHR systems like Athenahealth or eClinicalWorks is preferred. Demonstrated ability to work with individuals with disabilities or developmental challenges, including Alzheimer's or dementia care. Knowledge of advanced nursing practices such as pain management, infection control, ventilator management, and acute pain assessment. Experience in emergency medicine or urgent care environments that require quick decision-making and triage skills. Strong understanding of medical terminology, anatomy knowledge, ICD coding (ICD-10/ICD-9), CPT coding for procedures like suturing or spinal taps is advantageous.
