Job Overview We are seeking a motivated and compassionate Licensed Practical Nurse (LPN) to join our team in Assisted Living & Memory Care communities. In this vital role, you will deliver high-quality nursing care tailored to the unique needs of our residents, including those with Alzheimer's disease and other forms of dementia. Your expertise will support residents' health, safety, and well-being while fostering a warm, supportive environment. Duties Provide direct nursing care to residents, including medication administration, wound care, and vital signs monitoring Conduct comprehensive patient assessments and follow individualized care plans in collaboration with the healthcare team Manage acute pain and assist with complex medical conditions such as infectious disease care and post-acute needs Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Residex while maintaining HIPAA compliance Assist with mobility support using equipment such as Hoyer lifts and ensure proper catheter care. Collaborate with interdisciplinary teams on discharge planning, and case management. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in Minnesota Prior experience working in assisted living, memory care, or long-term care settings; experience with Alzheimer's/dementia care highly preferred Strong assessment skills with the ability to recognize changes in resident condition promptly Experience with medication administration, sterile techniques, and pain management protocols Excellent communication skills with a focus on patient-centered care; ability to work with individuals with disabilities or developmental challenges Commitment to maintaining HIPAA standards and infection control protocols This position will work under the AL/MC Director and RN and will be part of the departmental leadership team.
