Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise in patient assessment, medication administration, and clinical procedures will ensure our patients receive safe, respectful, and personalized care. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Assist with diagnostic evaluations such as specimen collection, medical imaging preparation, and laboratory testing. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory impairments, ensuring their safety and comfort. Support acute pain management strategies and emergency response procedures within hospital or urgent care settings. Maintain detailed medical documentation using electronic health record (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA and confidentiality policies. Collaborate effectively with multidisciplinary teams to coordinate discharge planning, case management, and client communication for optimal patient outcomes. Experience Proven experience working as an LPN in diverse healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks. Critical care experience in settings like Level I or Level II trauma centers or PICU is highly desirable. Familiarity with medical coding systems such as ICD-9 and ICD-10 for accurate documentation and billing purposes. Experience providing specialized care for geriatric patients with conditions like dementia or Alzheimer's disease; knowledge of memory care best practices is preferred. Skills in IV insertion, phlebotomy, tube feeding management, ventilator support, and sterile technique are essential. Knowledge of long-term care regulations and experience working with individuals with disabilities or developmental delays will enhance your effectiveness in this role.
