If you're tired of call lights, rushed medication passes, and feeling like there's never enough time for your patients, home health offers a refreshing change. As an LPN Visit Nurse, you'll care for one patient at a time in the comfort of their home—building meaningful relationships while practicing the clinical skills you worked hard to develop. This is nursing with purpose, autonomy, and flexibility. Why LPNs Are Making the Move to Home Health One patient at a time —no juggling an entire unit. Greater independence with the support of an experienced clinical team. Flexible full-time and part-time opportunities. Build lasting relationships with patients and families. Enjoy a role where your time is spent providing care—not answering call lights. A Few Perks We're Especially Excited to Offer Employer contributions toward medical insurance. Employer-paid dental, vision, life insurance, and disability coverage. Free access to our MedBridge Education Hub for continuing education and professional development. Licensure reimbursement when it's time to renew. Annual scrub reimbursement. Mileage reimbursement between patient visits. Cell phone reimbursement for eligible clinicians. Three weeks of PTO plus six paid holidays. Structured orientation, mentorship, and ongoing clinical support. Opportunities for career growth throughout our family of healthcare companies. Why Home Health Care, Inc. For more than 30 years, we've helped Minnesota families receive exceptional care where they feel most comfortable—at home. We're proud to be locally owned, Medicare-certified, and known for putting our clinicians first. When you join our team, you're backed by experienced Clinical Managers, supportive leadership, and coworkers who genuinely care about your success. What You'll Do Provide skilled nursing visits in patients' homes. Perform wound care, medication management, and other skilled nursing interventions. Educate patients and family members on treatments, medications, and disease management. Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team. Document visits accurately and ensure continuity of care. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license. Ability to work independently while providing compassionate, patient-centered care. Reliable vehicle, valid driver's license, and current auto insurance. Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred. EMR experience is a plus, but we'll help you learn our systems. Experience as an Army 68C , Navy Hospital Corpsman (HM) , Air Force 4N0X1 , Marine Corps HM , or Coast Guard Health Services Technician (HS) translates well into this role.
