Job Description

Job Overview Are you an LPN/BHP looking to join a team environment? Refractions IRTS is seeking passionate LPN/BHPs to make a real difference in the lives of adults. You will work directly under our two RN's supervision and help foster an environment where our adult clients can thrive. We are looking for part-time LPN/BHPs. If you enjoy working with individuals on a daily basis, this may be the opportunity for you. A highly qualified candidate will be eager to help clients learn the skills and tools necessary to live fulfilled and independent lives. We seek self-motivated individuals who enjoy teamwork as well as working independently. A qualified candidate must be able to collaborate effectively with our internal team and external stakeholders to support the individuals we serve. A valid driver's license is required, along with proficient case management skills, good writing skills, and attention to detail. Responsibilities Provide direct patient care under the supervision of our registered nurses. Assist in discharge planning to ensure smooth transitions for patients leaving the facility. Administer medications and monitor patients for adverse reactions Collaborate with healthcare teams to develop and implement patient care plans. Maintain accurate patient records using Procentive. Ensure compliance with all healthcare, regulations and 245I standards. Requirements Valid LPN license in the state of practice. 4000 hours working with adults who suffer from a mental health diagnosis, addiction, or traumatic brain injury, or 2000 hours and a bachelor's degree in a related field. Experience in an IRTS is preferred but not required. Knowledge of discharge planning processes and medication administration protocols. Proficient understanding of accurate documentation. Strong communication skills and the ability to work collaboratively within a team environment. Compassionate approach to patient care, with a focus on empathy and support.
