Licensed Practical Nurse (LPN) Job Description - Long-Term Care Facility The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for promoting residents' health, comfort, and well-being by delivering compassionate, high-quality nursing care while complying with all federal, state, and facility regulations. Essential Duties and Responsibilities Administer prescribed medications and treatments according to physician orders. Assess residents' physical and emotional conditions and report changes to the RN or physician. Monitor and document residents' vital signs, pain levels, and overall health status. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Provide wound care, catheter care, tube feeding, and other skilled nursing procedures within the LPN scope of practice. Supervise and delegate appropriate tasks to Certified Nursing Assistants (CNAs). Ensure accurate and timely documentation in residents' medical records. Respond promptly to resident emergencies and initiate appropriate interventions. Educate residents and family members regarding medications, treatments, and health conditions. Maintain infection prevention and control standards. Communicate effectively with physicians, nurses, therapists, residents, families, and other healthcare professionals. Promote residents' dignity, privacy, and independence. Participate in shift reports, care conferences, and quality improvement activities. Ensure compliance with all facility policies and applicable state and federal regulations. Qualifications Current, unrestricted Licensed Practical Nurse (LPN) license. Current CPR/BLS certification. Previous experience in long-term care, rehabilitation, or skilled nursing is preferred. Knowledge of geriatric nursing practices and resident-centered care. Strong communication, organizational, and critical thinking skills. Ability to work effectively as part of a multidisciplinary healthcare team. Physical Requirements Ability to stand, walk, bend, lift, and transfer residents as needed. Ability to lift up to 50 pounds with or without assistance. Ability to respond quickly to emergencies. Ability to perform repetitive tasks and work extended shifts when necessary. Work Environment The LPN works in a long-term care setting that provides 24-hour nursing services. This position may require working days, evenings, nights, weekends, holidays, and on-call shifts.
