The ideal candidate will possess a strong foundation in patient assessment, medication administration, and clinical procedures, with experience working with diverse patient populations, including geriatric and individuals with disabilities. As an LPN, you will play a vital role in supporting patient well-being, ensuring adherence to safety protocols, and collaborating effectively with multidisciplinary teams to deliver high-quality care. Duties Perform thorough patient assessments, including vital signs, physical examinations, and documentation of health status using electronic health record (EHR) systems. Administer medications and injections accurately while adhering to HIPAA and clinical confidentiality policies. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or those requiring memory care or hospice support. Support acute and chronic pain management through appropriate interventions and patient education. Collaborate with physicians and healthcare teams to develop and implement individualized care plans for patients assisted living communities, hospitals, and outpatient clinics. Conduct patient education on health coaching, medication management. Ensure compliance with long-term care regulations, infection control standards, and safety protocols during all patient interactions. Experience Proven experience as a Licensed Practical Nurse in diverse healthcare environments such as hospitals, nursing homes, or outpatient clinics. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or ECP for accurate medical documentation. Experience working with individuals with developmental disabilities or in specialized areas such as geriatrics or behavioral health is preferred. Knowledge of
