Job Summary :
Coordinates others ensuring completion of all Minimum Data Set (MDS) requirements in compliance with State and Federal Regulations governing the Resident Assessment process, within required time frames and criteria, including the implementation of CAT's and CAA's; must perform and tolerate a busy work pace; must communicate courteously and respectfully with Residents, Employees and others; must sustain the cooperation of others
Qualifications :
LPN license in good standing in the State of Mississippi; five years clinical experience, preferably 3 years in long-term care with at least 2 recent years in MDS experience; must have thorough knowledge of Medicare and Medicaid with comprehensive knowledge of Nursing techniques and their relation to medical practices, skills and their application, with thorough knowledge of principles and practices of Long-Term Care, to include infectious diseases and aseptic techniques, including Universal Precautions; through knowledge of State and Federal Regulations for Long-Term Care; basic computer skills in Microsoft Office, good oral and written communication skills; must currently possess, or attain TB Certification and MDS Certification within first year of accepting position and must receive MDS Recertification as required; must pass drug screens and have a satisfactory
Criminal History Background Check Duties and Responsibilities:
Tracks due dates for MDS; coordinates and conducts development and completion of Resident Assessment Process in compliance with company policies, State and Federal Regulations Ensures compliance/completion of the MDS within pre-set time frames and criteria Assists in the Admissions process that includes Advanced Care Planning and a Baseline Care Plan; assists in the development of a comprehensive Resident assessment and Care Plan for each Resident as Chairperson of the facility's Interdisciplinary Care Plan Team Will be a part of discussing discharge goals and discharge planning with resident/resident representative on admission
