Job Description

LPN (Licensed Practical Nurse)Job Summary We are seeking a compassionate LPN (Licensed Practical Nurse) to provide direct nursing care and ensure resident safety in a long-term care setting. The Licensed Practical Nurse (LPN) will work as part of an interdisciplinary team, supervise nursing assistants, and deliver high-quality, resident-centered care. Key Responsibilities Provide direct resident care, including medication administration and treatments per physician orders. Monitor and assess residents' physical and emotional status and report changes promptly. Supervise and support nursing assistants with daily assignments and care delivery. Respond to resident incidents, emergencies, and follow reporting procedures. Maintain accurate clinical documentation and contribute to care plans. Uphold resident rights, privacy, dignity, and confidentiality. Follow all safety, infection control, and regulatory guidelines. Requirements Graduate of an accredited Licensed Practical Nurse program. Current, active LPN (Licensed Practical Nurse) license in this state. Strong communication, organization, and clinical judgment skills. Ability to work effectively with residents, families, and care teams.
