Position Summary Sun River Health & Rehab is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our clinical team. Full-time and part-time opportunities are available. As an LPN, you will play a vital role in providing high-quality, resident-centered care while supporting the physical, emotional, and psychosocial well-being of our residents. Working closely with physicians, Registered Nurses, Certified Nursing Assistants, and other healthcare professionals, you will help ensure that each resident receives individualized care in accordance with physician orders, care plans, and regulatory requirements. The ideal candidate is compassionate, organized, detail-oriented, and committed to delivering exceptional care to seniors and individuals requiring skilled nursing services. Essential Duties and ResponsibilitiesResident Care Provide direct nursing care to assigned residents in accordance with physician orders, individualized care plans, and nursing standards. Deliver person-centered care that respects resident rights, preferences, goals, and choices. Monitor residents for changes in condition and promptly communicate concerns to physicians, supervisors, and family representatives as appropriate. Review and implement resident care plans and update interventions as needed. Medication & Treatment Administration Administer medications and treatments accurately and safely according to physician orders. Document all medications, treatments, observations, and resident responses in the electronic medical record. Monitor residents for medication effectiveness, adverse reactions, and changes in condition. Maintain medication carts, treatment carts, and medication storage areas in compliance with facility policies and regulatory requirements. Order and reorder medications and medical supplies as needed. Clinical Documentation Complete assessments, progress notes, incident reports, and required documentation accurately and timely. Enter and process physician orders according to facility procedures. Maintain complete and compliant medical records for assigned residents. Document refusals of care and follow facility protocols for notification and follow-up. Admissions & Discharges Assist with resident admissions, readmissions, and discharges. Complete admission assessments, obtain physician orders, review documentation, and orient new residents to the facility. Prepare discharge paperwork, medication instructions, and resident education materials as appropriate. Communication & Care Coordination Participate in shift-to-shift report and ensure continuity of resident care. Complete controlled substance counts and reconciliation during shift changes. Collaborate with physicians, pharmacy providers, laboratory services, therapy teams, and other healthcare partners. Communicate effectively with residents, families, and interdisciplinary team members. Safety & Quality Assurance Initiate and complete incident reports, assessments, and required notifications when necessary. Monitor resident safety and identify opportunities to improve care quality. Participate in facility surveys, audits, inspections, and quality improvement initiatives. Follow infection control procedures and all safety protocols. Team Support Supervise and delegate appropriate nursing tasks to Certified Nursing Assistants and other support staff in accordance with state regulations and facility policies. Assist in maintaining clean, organized work areas, nursing stations, supply rooms, and resident care areas. Attend staff meetings, in-service training, and continuing education programs. Perform additional duties as assigned. QualificationsRequired Graduate of an accredited practical nursing program. Current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice. Current Basic Life Support (BLS) certification or ability to obtain upon hire. Ability to successfully complete required pre-employment screenings, including criminal background checks. Strong communication, organizational, critical thinking, and time management skills. Ability to use electronic medical records and other healthcare technology systems. Preferred Previous nursing experience in a skilled nursing facility, long-term care center, rehabilitation facility, hospital, or similar healthcare setting. Experience caring for geriatric, rehabilitation, dementia, or medically complex residents. Physical Requirements Ability to stand, walk, bend, lift, and assist residents throughout the shift. Ability to lift, transfer, push, or carry items exceeding 30 pounds when necessary. Ability to respond quickly to resident care needs and emergencies. Ability to work in a fast-paced healthcare environment while managing multiple priorities. Ability to work with residents experiencing dementia, chronic illness, mental health conditions, and disabilities.
