Job Description

Carolina Meadows, located in Chapel Hill, North Carolina, is one of the nation's foremost continuing-care retirement communities. Carolina Meadows is in search of a Full Time Nurse Coordinator-LPN primarily responsible for providing direct medical care to patients of the Medical Practice of Carolina Meadows while ensuring the highest level of care and satisfaction is provided.
Hours:
Monday-Friday 8:30am-5:00pm This position is primary responsible for providing direct medical care to residents of Carolina Meadows while ensuring the highest level of care and satisfaction is provided.
Requirements:
Completion of Practical Nursing Program, and two (2) to four (4) years of long-term care and geriatric medical service experience preferred. Must also possess high-quality verbal and written communication skills, strong customer service skills and must be proficient in the operation of computer applications, general office and medical equipment. Medical Care Performs necessary nursing duties to maintain the highest level of patient service and satisfaction. Accurately collects information from residents to include chief complaint, history of present illness, any current medical and/or social problems, current medications, allergies and vital signs; recognizes emergency situations and addresses appropriately. Performs focused physical examination of patient; recognizes and coordinates needs for further medical evaluation and treatment. Coordinates care of residents with primary physician, nurse practitioner, other medical providers and family members. Educates residents on their health to include conditions, diseases, medications, diet, etc. as directed. Manages wound care; communicates needs and coordinates care with primary medical providers and consultants. Stocks clinic with medical supplies and drugs when delivered and submits list of necessary items as needed. Completes resident lab work to include drawing the sample, completing the requisition form and placing blood, stool, and sputum samples with form to be sent and processed in designated lab collection box. Ensures all tests are picked up by the courier and sends out labs on STAT when appropriate. Follow up on lab values and new orders as directed by medical provider. Takes and transcribes physician orders. Provides PPD testing for residents and staff as directed. Administers therapeutic injections and immunizations as ordered by providers. Obtains EKG's, PVR with bladder scan, Oxygen walk tests, and assists with Foley catheter care as directed. Participates in community response triage following established Carolina Meadows protocol. Assists medical providers during examinations as requested. Administrative Functions Performs clerical functions for clinic as well as other general office duties as assigned. Updates medication lists and refills prescriptions as directed by the provider in a timely manner. Monitors S-Fax daily as directed. Assists providers with patient referrals and prior drug authorization requests as directed. Ensures exam rooms are adequately stocked and prepared prior to patient hours, contributing to the maximum efficiency of the practice. Ensures all resident charting and nurse billing are completed in an accurate and timely manner
