Clinical Call Line Nurse (LPN) Job Summary The Clinical Call Line Nurse (LPN) serves as a centralized clinical resource responsible for managing incoming patient and parent telephone communications, providing standardized clinical guidance within scope of practice, coordinating patient care, and facilitating timely provider communication across Jacksonville Children's and Multispecialty Clinic. Working under established clinical protocols, standing orders, and provider direction, the Clinical Call Line Nurse assesses patient concerns, identifies urgency, coordinates appropriate disposition, and ensures accurate documentation within the electronic health record (EHR). This position plays a critical role in improving patient access, supporting providers, reducing interruptions to clinic workflow, and promoting safe, consistent clinical communication across all practice locations. In addition to telephone triage responsibilities, the Clinical Call Line Nurse serves as a clinical support resource by assisting with nurse visits, high-level clinical procedures within scope of practice, provider support activities, quality initiatives, and other operational needs as assigned.
Key Responsibilities:
Clinical Call Coordination Manage incoming patient and parent clinical telephone calls from the Pediatric Well clinic. Verify patient identity and maintain HIPAA compliance. Assess the reason for the call using standardized clinical workflows and approved telephone triage protocols. Determine urgency based on approved organizational guidelines. Route patients appropriately to emergency care, same-day appointments, provider review, or routine follow-up. Recognize situations requiring immediate provider involvement or emergency referral. Provide standardized patient education and guidance using approved scripts and clinical resources. Escalate all concerns requiring diagnosis, treatment recommendations, medication management, or clinical judgment beyond scope of practice. Manage voicemail queues and ensure timely follow-up of patient messages. Coordinate communication between providers, patients, pharmacies, hospitals, specialists, home health agencies, and ancillary services. Complete all telephone documentation accurately within the EHR. Clinical Coordination Process clinical tasks, messages, and provider communications. Assist with laboratory result routing and patient notification as directed by providers. Coordinate medication refill requests according to organizational workflow. Support prior authorization and referral communication when clinical input is required. Assist with newborn hospital discharge scheduling according to organizational policy. Facilitate communication regarding diagnostic testing, follow-up appointments, and care coordination. Direct Clinical Support The Clinical Call Line Nurse may: Perform nurse visits and other delegated clinical services. Assist providers with procedures requiring licensed nursing support. Perform urinary catheterization and other approved procedures within scope of practice and organizational policy. Support vaccine, medication, and treatment administration as assigned. Assist with clinical quality initiatives, preventive care outreach, and population health activities. Support clinic staffing needs during periods of increased patient volume. Qualifications Education High School Diploma or GED required. Graduate of an accredited Practical Nursing program. Licensure Current North Carolina Licensed Practical Nurse (LPN) license in good standing. Current BLS Certification required. Experience Minimum of two years of clinical experience preferred. Previous experience in pediatrics, family medicine, urgent care, telephone triage, or primary care preferred. Skills Strong clinical assessment and critical thinking skills. Excellent telephone communication and customer service skills. Ability to prioritize multiple patient concerns in a fast-paced environment. Knowledge of pediatric and adult ambulatory care. Strong documentation skills. Ability to work independently while recognizing when provider escalation is necessary.
